How to Choose the Right Acne Scar Treatment and Why It Matters MoreThan You Think

Acne scars are one of the most common concerns I see in my Needham practice, and also one of the most undertreated. Many people spend years covering them with makeup or simply assuming there’s nothing to be done. That’s not true, and you deserve better information than that.

The good news: there are more effective options than ever. The important caveat: not every treatment works the same way on every scar type or skin tone. What works beautifully for one patient may not be the right starting point for another. That’s why at WAVE Medical Aesthetics, we always begin with a thorough consultation before recommending anything.

Here’s what you need to know before booking a treatment.

First: Not All Acne Scars Are the Same

This distinction matters more than most people realize, because scar type directly determines which treatments will be effective.

Ice pick scars are narrow and deep, almost like a small puncture in the skin. They’re stubborn and often require targeted approaches.

Rolling scars create a wavy or undulating texture across the skin surface, caused by fibrous bands tethering the skin below.

Boxcar scars have sharp, defined edges and can vary in depth. They’re common on the cheeks and temples.

Most patients I see have a combination of types, often mixed with post-inflammatory hyperpigmentation (PIH), the darkening that lingers after a breakout. PIH isn’t a scar in the structural sense, but it absolutely affects how the skin looks and needs to be factored into your treatment plan.

What “The Best Treatment” Actually Depends On

There isn’t one single “best” acne scar treatment, and anyone who tells you otherwise is oversimplifying. The right choice depends on a few things we assess together:

  • Your scar type and depth. Structural scars (rolling, boxcar) respond differently than surface texture irregularities.
  • Your skin tone. Some energy-based devices carry a higher risk of post-treatment pigmentation in deeper skin tones. Choosing the wrong laser can make things worse, not better.
  • Your downtime tolerance. Some treatments involve a few days of redness and peeling; others are genuinely low-disruption. Both can be effective. The question is: what fits your life right now?
  • Whether you’re dealing with active breakouts. We address skin stability before we address scars. Treating scars on actively congested skin is counterproductive.

Secret™ RF Microneedling: Our Core Treatment for Structural Scars

For most patients presenting with rolling or boxcar scars, Secret™ RF Microneedling is where I start. It’s one of the most well-researched tools we have for collagen remodeling, and collagen remodeling is the mechanism that actually changes scar structure over time.

How it works: microneedles create controlled micro-injuries at precise depths, triggering the skin’s healing response. Simultaneously, radiofrequency energy is delivered into the deeper dermal layers to stimulate collagen and elastin production. Over a series of treatments, patients see smoother texture, softened scar edges, and skin that reflects light more evenly.

I want to be honest about expectations: RF microneedling doesn’t erase scars – no treatment does that reliably. What it does is meaningfully improve them, with results that continue to develop for months after treatment as collagen matures.

Downtime is minimal, typically 24–48 hours of redness and mild swelling, and most patients return to normal activities the next day.

Fraxel Dual: For More Aggressive Resurfacing

For patients with more pronounced scarring, or those who also want to address tone and overall texture in a more comprehensive way, Fraxel Dual is a powerful next step. It’s a fractional laser that creates microscopic treatment zones in the skin, triggering resurfacing and deeper collagen remodeling simultaneously.

Fraxel requires more downtime than RF microneedling (typically 5–7 days of redness and peeling), but the tradeoff is a more noticeable change per session. It’s particularly well-suited for patients with mixed scarring and uneven tone, or those who have already completed a course of RF microneedling and want to take results further.

Appropriate settings vary by skin type and Fitzpatrick scale, something we always discuss in detail before any Fraxel treatment.

Supporting Treatments: Peels, IPL, and Laser Genesis

Structural scar treatments address depth and texture, but many patients also deal with residual discoloration, dullness, and visible pores. Depending on what I see at your consultation, I may recommend layering in additional options:

Laser Genesis (Xeo®) is a gentle, non-ablative option that works well for redness and overall skin quality, with zero downtime.

IPL Photofacial targets pigmented and vascular irregularities, useful when acne has left behind a mix of brown spots and redness.

How Many Sessions Should You Expect?

Most patients doing RF microneedling see noticeable improvement within 3–4 sessions, spaced 4–6 weeks apart. Fraxel may achieve more dramatic change in 2–3 sessions but with more downtime per treatment. Combination approaches can sometimes compress the overall timeline.
